Apply for a node-operator account

Request one node-operator role — exactly one (tier, node) pair per request. A Continental Infrastructure Admin can grant additional roles for other nodes after approving this one. Registering a brand-new Sub-regional Hub, National Hub, or Spoke that doesn't exist in the federation yet? Do that first at /register-node.
